[feat] Put higher accent on doctor-patient collab

To avoid any hallucination issues we could provide a workflow where the results still could be uploaded by patients (and doctors) but AI Chat is limited to doctors only and results are published to patient only after doctors approval

Still requires some investigation with the actors - doctors mostly
