Clean up and fix UI to improve responsiveness

Here and there we have custom-coded snippets that don't follow Bootstrap layout/desing and hence is poorly responsive, e.g:

1) menu bar: it does collapse, but first it unexpectedly "breaks" in 2 lines;

2) "profile" runs out of the view and buttons are missaligned:

and so on...

The "core" functionality could be made to be accessible from mobile devices...

Likely a few places where this is true... @AlexFreemantle ? A worthwhile exercise to track down where UI oddities remain, e.g, the task approval confirmation.
