Recipe on adding/modifying chapters

I received a request via email (perhaps they do not have a GitHub account): They would like "some working examples on how to use (add/modify, etc.) Chapters in mp4 and mkv files."

Does anyone have a script in mind, or something they've used in the past?

I will try to come up with something and add it, but I haven't personally done much here.
