It would be great to have "each" passing "index" along to the iterator function.
Right now iterator is called as iterator(item, callback). Which could become iterator(item, index, callback).
iterator(item, callback)
iterator(item, index, callback)
The same concept could be applied to sister functions like "eachSeries" and "eachLimit". What do you think?
