Virtual documents aren't tracked. #2431

Basically C# documents that don't exist on disk aren't tracked/updated as part of a normal C# project.

I'll write up the code for this guy and put out a PR sometime next week.

Track virtual documents.

- Track opening, closing and changing of virtual documents that don't exist on disk.
- Updated the various document selectors to properly include virtual files.
