When importing a MIDI file from the Media Explorer, a first window opens which asks us this:
Expand 24 MIDI tracks to new tracks REAPER check box checked
I press Tab to the OK button and press Enter or Space.
Another window appears that says this:
Import 14-channel MIDI and meta events as: Single-channel objects drop-down on multiple tracks collapsed
I press Tab to the OK button and press Enter or Space.
Now I'm supposed to go back into the TCP but nothing, even if I press the up or down arrow, I no longer have any feedback from NVDA.
At this point, if I undo the last action with Ctrl+Z and redo with Ctrl+Shift+Z, I regain control in the TCP and I can see all the tracks as well as the return from NVDA.
On the other hand, if I copy the MIDI file from Windows Explorer and I paste the file into TCP, I have the same 2 windows that appear and when I press Enter or Space on Ok in the second window, I go back to TCP and everything works perfectly.
I tested with several MIDI files on 2 different machines on Windows 10 Pro with Reaper 7.22 with the latest version of OSARA and I have the same behavior.

I think the behavior you describe is normal, since in the latest versions of reaper, fortunately, the focus does not move from the media browser when you insert an element, so if the focus remains on the media browser you cannot see the tracks, to see them you can try either to escape or to close the media browser with the control alt X key combination.
