Failure to create correct slug (id value) after Markdown-it Typographer

The problem is: this plug-in does not work if typographer is enabled (very typical) and there are " ' += ..., etc. in some header.

To reproduce:

Use input, with Typographer enabled:

# Some Heading...

It would not be a problem but TOC generator (your plug-in is only needed when a TOC generator is used) will usually work before Typographer. Id values will mismatch, the navigation won't work.

I repeat: separate "named-header" and TOC plug-ins make no sense. Both functions should be performed by a single plug-in. Okay, I guess I'll do it.
