Release for DIHARD II

This is an update for use by DIHARD II. Changes for this release:

• addition of Jaccard error rate (JER) metric, which is output under the JER column in the scoring table
• substantial source code clean up
• unit tests for code modules

This is a bugfix release that fixes the -S flag, which previously would default to the same value as the -R flag. This is the release that was used for scoring of system outputs during the First DIHARD Challenge and should be used by all papers attempting to compare against those results.
