Entrust CA is listed in supported CA list but does not appear in list of ACME endpoints in the deployment template

Describe the bug
While Entrust CA is supported by acmebot, the Entrust ACME service's URL does not appear in the available list when deploying acmebot.

To Reproduce
Steps to reproduce the behavior:

1. Click on "Deploy to Azure" from the project's github page
2. Note that [acme.entrust.net/](https://acme.entrust.net/acme2/directory is not listed.
