As a developer I want to use audio streams as they are defined by BAP, where they are only unidirectional.

Remove the BT_CODEC_QOS_INOUT, so that a single stream cannot be configured for both audio in and audio out.

(CISes can be bi-directional - add support for two BAP streams sharing a CIS stream later.)

Remove macro, update LC3 defines
