Meteor.default_server was undocumented, and finally removed in Meteor 2.3

Meteor.default_serveris undocumented, and was finally removed in Meteor 2.3.This causes this package to be incompatible with Meteor >2.3.
Note: I'm inexperienced in developing Meteor packages, so I haven't tested that this actually works.
