Survey table ui improvements

• Put buttons in a triple-dash menu so they can have some text as to what they do
• Improve active/inactive indicator to a round indicator with text

Rename active/inactive to included_in_simulation
Separate label with action.
Label as "Included in Simulation"
Display a green or red dot to indicate whether it is included in the simulation.
Move the action to include or remove the simulation to the triple-dash menu.
When "Include_in_simulation"==true => Display in the menu "Remove to the simulation"
When "Include_in_simulation"==fals => Display in the menu "Add to the simulation"
