Generate markdown documentation on the fly

Closes #16 by switching from hard-coded Markdown documentation files for instructions and assemblers to dynamically generated documentation. This is about 300x faster, but more importantly will allow programmatic access to documentation (e.g. opcode parameter lists) for #5.
