Issue description

Currently, the metadata report for domain requests does not reflect the correct data for requests that originate from a portfolio. We need to update the output of that report.

This report can be accessed via the Django admin > Analytics dashboard.

Acceptance criteria

• Update the report to reflect the 5 new columns highlighted in yellow within this document.
• The order of the columns should match the order they are listed in that document.
• Ensure existing fields in the report are pulling the correct data for requests that are linked to a portfolio. (e.g. Domain Type is pulled from the portfolio rather than the domain request).
