This is going to get stomped on slightly by a preceding patch that adds init/quit support, so we don't need to worry about getting this in sooner anymore. |

This consolidates dxvk_platform_exts to a single file, which references wsi_platform to get the instance extensions via WSI-specific APIs. Most of this patch is just moving the platform-specific stuff from one file to another.
This makes it a lot easier to implement #3736!
