Refactor ippproxy code:

- Now supports Register-Output-Device when using a System URI.
- Better handling of error conditions.
- Use newer CUPS 3.x APIs.
- Fix some potential deadlock issues.
- Fix handling of missing number-of-documents value.
- Fix Update-Document-Atributes call (wrong group for output-device-document-state)
