v2.0.0 Release

This release contains the following changes:

• Grafana has been upgraded to version 5.1.3.
• The initial setup process has been greatly simplified thanks to the provisioning features that were introduced in Grafana 5.
• The Mikrocount app will (hopefully) no longer record data for IP's that are not within the designated subnet.
