Fix cases where level 3 is worse than level 2, by inserting intermediate hashes on skipped sections and increase level 3 table size.

Fix most cases where level 3 is worse than level 2, by inserting intermediate hashes on skipped sections and increase level 3 table size.
Also fixes immediate matches not being caught.
