Ensure to always use "blank" target for external links

them. This includes a new layout and structure, better and clearer

Fix invalid image paths after asset restructuring
819d325 
GH-135 included some changes to the structure of the project assets.
The `README.md` and `CHANGELOG.md` files were not been updated
accordingly resulting in a invalid image path for the repository hero
that is now placed within the `images` subdirectory.
