The challenging part of this will be finding a scalable way store all of this information. With this issue we may need to consider moving to a custom table rather than the WP options interface.

As of milestone 4.0, the cache supports multiple dimensions of thumbnail per attachment. It's just a matter of exposing this from the frontend.
