Fix bug where outline is opened before it's populated

• Fixed a bug where the outline would sometimes error out because it was being opened before it was populated

• Changed the outline to show the embedded note preview by default

  The long-term plan is to make this an option via the settings so you can choose whether to use the embedded note preview or just a link to the note.

Fix folder bug

• Fixes a bug where the outline wouldn't correctly account for folders when traversing related notes
• Plugin now opens the generated outline after use
