Description
I have addressed this issue successfully by adding the 'Rate Us' menu on the Nav Bar of all the pages:

Contact Us
Offers
Login
Sign-up
Cart
This will allow the users to easily navigate to the 'Rate Us' page.
Fixed Active Nav
I also fixed the active nav on the Rate Us page.
